HMS Osiris was the third vessel to bear this name.

Osiris made a return visit to Barrow in December 1969 when Lt-Cmdr E.S.J Larkin signed the visitors' book at Barrow Town Hall. The boat's captain had stood-by the nuclear suibmarine Valiant as navigation officer during the Barrow build and had lived at Canal Foot, Ulverston. Osiris was opened to the Barrow public for two days and had 50 visitors.

During the Falklands War of 1982, HMS Osiris was employed on minelaying work in the South Atlantic. The boat's longest deployment was a Falklands patrol in 1984. Her crew also held a record for rescaping from a submarine when a series of deep escapes were carried out off Malta in 1970.

In 1988 Osiris was fitted with the latest type of sonar equipment during a refit at Devonport - the first to be completed commercially in a Royal dock yard.

When Osiris was decommissioned in 1989 she was and sold to Canada for use as spare parts. The Osiris was towed to Birkenhead on the River Mersey where Cammell Laird shipyard completed the stripping out process. In August 1991 the remains were moved to Garston for final demolition and scrapping in 1992.
